As part of this transition we are changing our name, effective May 21, to Southeast Correctional Medical Group, or SECMG. SECMG will continue to be a locally-focused, locally-led company, proudly serving our client family in Georgia, North Carolina and throughout the Southeast.

Our Story

TransformHealthCS has been delivering clinical based healthcare services since 2003. We currently do business in Georgia, Tennessee, and North Carolina. Our management team has strong backgrounds in clinical and healthcare administration services. We are clinically focused and innovation driven.

What we believe in..

We believe in community engagement. We emphasize the importance of getting our staff to co-ordinate their efforts locally and face-to-face with everyone in the care process. Our philosophy says “It takes boots on the ground, Eye to Eye patient contact to change behaviors”. We understand the challenges that counties face with rising medical cost, rising pharmacy cost, and the difficulty in managing health issues created by patient lifestyles.

We believe in applying technology to get specific results. Our Dashboard system allows all stake-holders to continually monitor the progress of our programs focused on cost savings, utilization, and patient outcomes. We strive for a eco-friendly paperless environment using an internet based Electronic Health Record and reporting system. We also use video conference technology to communicate internally between our clinics to encourage best practice exchange.

We believe in partnering with our industry stakeholders and taking a leadership role in reforming the corrections healthcare delivery system.

C.H.U.M.P.S training class .

Nurses from several of our Georgia correctional facilities participated in a C.H.U.M.P.S training class hosted by Warden Spears at Effingham County Prison. Unit Manager Frazier taught the class, which was geared toward inmate manipulation. The informative class was an eye opener even to our veteran correctional nurses.
